Could i have possibly have anemia?

I feel tired all the time. I get lightheaded when i stand up often. I get dizzy at times. I was barely able to donate blood because my hemoglobin level 12.5. I blacked out after donating before. Last time i just got woozy, lightheaded and dizzy. I get out of breath even just walkin an goin upstairs. I get chest pains at time and rapid heartbeat. Please anyone who can help. Is it worth goin to doc to check out?
